This portion of the rotation will consist with shadowing and assisting a consultant long-term care pharmacist who works with Causey's Pharmacy.  You will have the opportunity to visit nearby nursing homes in the community and experience drug regimen reviews (see below), emergency kits, and the importance of maintaining medicine rooms and charts.  It will be vital you read two articles prior to working Mr. Barnhill.  Part of this experience will be collecting data on one patient to present a case presentation. The presentation will consist of a detailed past history and current present history.   The format for the case presentation will be in Xavier's PEP manual.  The evaluation form will be provided on your first day of rotation from Dr. Boyd.

Drug Regimen Review Checklist

Drug Indications
    1. Does each prescribed medication have a current and valid indication?
    2. Does the resident have conditions or indications for which medications may be appropriate but are not being used?

Medication Effectiveness

  1. Is the medication appropriate for the indication being treated?
  2. Is the dose of medication adequate?

Medication Safety

  1. Is the dose of medication excessive?
  2. Is the resident experiencing signs or symptoms of adverse medication effects?
  3. Is the resident experiencing a problem resulting from a drug-drug, drug-food, or drug-laboratory test interaction?
